Samenvatting van het onderzoek

Rationale: care for patients with Chronic Kidney Disease (CKD) necessitates a tailored pathway between primary and secondary care. Optimal use of secondary and primary care facilities requires the balance between timely consultation and referral but also timely referral back to primary care. It is not known what factors could facilitate a safe shift from secondary to primary care in managing CKD patients.

Objectives:  to study how referral back from secondary to primary care can be facilitated by a (web-based) consultation system and can comprise patient participation.

Study setting and design: a pilot study in CKD patients who are referred from secondary to primary care, where they receive care commissioned by the nephrologist. Patients will be recruited from the Canisius Wilhelmina Hospital (CWZ) and will be treated during follow-up by their own general practitioner.  

Study population: CKD patients who are under nephrologists’ care and meet the Criteria to Refer patients Back from secondary to primary care. These criteria are formulated in a local consensus meeting with nephrologists and based on (inter)national guidelines.

Data analysis: satisfaction and suggestions concerning referral back to primary care by patients, nephrologists and General Practitioners (GPs) will be evaluated by questionnaires. After one year we will monitor crucial parameters (renal function, blood pressure, etc) and treatment that patients have received.  Furthermore we count the patients who remained in primary care and investigate reasons for re-referral from primary to secondary care and nephrologists’ involvement during follow-up.
